60 years old lady himachali by origin had a non healing ulcer for six months ,which was increasing in size. Edge biopsy done came as squamous cell carcinoma . She was taken up for resection and abdominal wall reconstruction. Whole tumour was excised with 3 cm margin all around. Excision was till fascia , and flap cover was done. Rotation flap was done . She developed swelling in her lower jaw.   Biopsy of swelling came as squamous cell carcinoma . She under went complete excision of lesion with clear margins and neck dissection. Pectoral major flap and skin was used to reconstruct the defect. Feeding tube was inserted for feeding up to a month.
