T4 oral cancer treatment

44 year old male with T4 oral cancer of buccal mucosa . It was small non healing ulcer in gingiva and it kept growing till it eroded cheek wall .  As it spreads tumor also goes into lymphatics resulting in neck nodes enlargement. pre op photo Marking for excision Excised specimen  inner aspect excised specimen outer aspect After 10days Donor site patient was planned for surgery complete excision of tumour was done and neck dissection was done. Reconstruction was done from PMMC flap . Tracheostomy was done to protect airway. After 5 days tracheostomy was removed and patient was discharged. Fungating neck ,neck dissection and Deltopectoral flap

Patient was diagnosed with carcinoma base of  tongue , and was advised chemotherapy and radiotherapy. Patient  somehow unable to undergo treatment ,developed neck nodes which burst open . Patient was operated for neck dissection where all fungating and other nodes were removed and deltopectoral flap was done to cover the wound. Patient was advised again chemotherapy and radiotherapy . She developed swelling in her lower jaw.   Biopsy of swelling came as squamous cell carcinoma . She under went complete excision of lesion with clear margins and neck dissection. Pectoral major flap and skin was used to reconstruct the defect. Feeding tube was inserted for feeding up to a month.
